Your Initial Appointment/Consultation

Please bring the following information to your appointment:

  • Your current insurance card(s)
  • A list of your current medications (both prescription and over-the-counter), including your dose and frequency
  • Information pertaining to your medical and surgical history
  • Copies of MRI’s, X-rays, CAT scans or other imaging studies
  • Photo ID
  • Your co-pay, as determined by your insurance carrier. We accept cash, Mastercard and Visa. No American Express please.

Your Appointment for a Procedure:

  • You will have been given instructions either in person or on the phone prior to your appointment
  • If you are receiving conscious sedation, a driver must be present prior to the procedure
  • Please update us if any medical history or medications have changed since your last visit
  • No swimming or exercise is recommended following your procedure for 24 hours.

Office Hours and Urgent Care

Office Hours are Monday through Friday 8:00 am - 5:00 pm
Urgent visits will be scheduled either same or next business day

We accept most major insurance plans and secondary insurances, including:

  • Workers Compensation
  • Personal Injury/Auto Insurances (PIP)
  • Self Pay

Please call our office to confirm that we still accept your insurance provider as the above list can change. It is also recommended that you check your insurance plan's provider list to be sure it allows you to be seen in our office. Please be aware that your insurance plan may require a referral from your primary care provider.
